Intake Form & Vaccination Record
The Intake Form & Vaccination is your pet’s medical record and is very important you complete it completely, and send to DogFeathers, Inc. before your scheduled grooming date. Please help us help you by sending the completed form to us via email at DogFeathersATL@gmail.com, or you can drop it off, fax it to us at 404-499-1007, or mail it to us at 189 North Clarendon Ave, Avondale Estates, GA 30002.
All cancellations MUST be at least 24 hours before your grooming appointment. We strictly enforce this policy. Thank you for your understanding.
Grooming Drop Off and Pick Up Hours
Grooming by appointment only, please. Please be aware of the pick up times and late fees as indicated below.
||Tuesday - Friday
||8:00 a.m. to 10:00 a.m.
||8:30 a.m. to 10:00 a.m.
||Appx. 3:00 p.m. until 5:30 p.m.
||Appx. 2:30 p.m. until 4:30 p.m.
In order of preference:
- Personal checks
- Debit Card
- MasterCard or Visa (sorry, but we do not accept American Express or Discover)
Fees and Refunds
- $10 cancellation fee (see above for details)
- $10 late fee charge for each ten (10) minutes after last pickup time unless prior arrangements are made.
- $30 returned check fee.
- $15 minimum Express Grooming fee (for grooming completed before 1:30 p.m.)
- $15 minimum charge for credit card purchases.
- Please honor your appointment. If you have three “no shows” we will need to increase your grooming fee.
- Sorry, no refunds. We will be happy to issue you a store credit.
Gratuities are completely at your discretion. If you also wish to express your appreciation with a tip and are paying by either debit or credit card, please state the amount of the gratuity at time of payment and it will be added to your total.
